MI = 1ST LINE OF DEFENSE

Ambe nak kabor sini, Mechanical intergrity adalah protection paling awal dalam 
dalam melindungi process facility. Dalam kitab PSM yang ambe rujuk , MI ni boleh
dicapai dengan "design, installed and operate it right". Kitab PSM tu juga 
menyatakan keperluan MI perlu terguna pakai untuk setiap peralatan seperti 
berikut:

i)                   Pressure vessels and storage tank;
ii)                Piping systems(including piping components such as valve);
iii)              Relief and vent systems and devices
iv)              Emergency shutdown systems;
v)                Controls(including monitoring devices and sensors, alarms, and interlocks); and
vi)              Pumps.

salah satu incident yang dahsyat disebabkan oleh failure 
of MI ni adalah seperti keratan berita berikut:

Catastrophic explosion in 
Mexico City November 19, 1984 
Six hundred people were killed and 7,000 injured from an explosion following the failure of containment at a Mexico LPG facility in November, 1984. The storage facility contained four 422,000 gallon spheres of LPG. A  line corroded over time and eventually failed. The spilled propane filled the concrete dikes, overflowed, found an ignition source, and exploded. The explosion damaged other spheres, triggering more explosions. Twelve cylindrical tanks were also damaged, some rocketing over 3,000 feet away. The blast was felt over 12 miles away.
satu lagi fakta menarik adalah ; "According to information published in 2003 by Marsh Insurance, approximately 46% of the largest insurance losses in chemical, petrochemical, petroleum refining, and natural gas processing facilities from 1972 to 2001 were due to a failure in mechanical integrity programs"
